A stakeholder register records who your stakeholders are and how to engage them, including interest, influence, and attitude. A RACI matrix is narrower, assigning task-level responsibility (Responsible, Accountable, Consulted, Informed) for specific deliverables.
Review it at every phase transition or gate review, and whenever a new party joins, a sponsor changes, or someone's influence or attitude shifts. On longer projects a monthly check keeps engagement levels accurate.
