About the Action Log template

An Action Log is a running record of specific tasks agreed during meetings or project work, capturing who owns each item, what needs doing, and when it is due. It turns discussion into accountable follow-through and stops commitments from slipping between status updates. Unlike a full RAID log, it focuses purely on discrete, assignable actions.

It's part of My QMS, MyPMP's Quality Management System: fill it in online, personalize it with your name and logo, then export a clean, branded PDF. Your work auto-saves in your browser.

When to use a Action Log

  • β–ΈRecording follow-ups from weekly team or stakeholder meetings
  • β–ΈTracking corrective actions from a project review or retrospective
  • β–ΈManaging small tasks that don't warrant a full work-breakdown entry
  • β–ΈChasing open items during daily stand-ups or steering committees

What a good Action Log includes

  • βœ“Unique action ID and short description of the task
  • βœ“Owner responsible for delivery
  • βœ“Date raised and target due date
  • βœ“Status (open, in progress, blocked, closed)
  • βœ“Priority or urgency indicator
  • βœ“Notes, updates, or dependency links

Tips & common mistakes

  • πŸ’‘Assign a single named owner per action, not a team, so accountability is unambiguous
  • πŸ’‘Review and update statuses at every meeting rather than letting stale items pile up
  • πŸ’‘Give each action a clear, verb-led description so it's obvious what 'done' looks like

How it works

  1. 1. Fill it in β€” type directly into the fields, tables and sections above.
  2. 2. Brand it β€” add your organization name and logo with the Branding button.
  3. 3. Export β€” print to PDF, or become a member to white-label and sync across devices.

FAQ

What is the difference between an action log and a RAID log?οΌ‹

An action log tracks only discrete tasks and their owners, while a RAID log captures risks, assumptions, issues, and dependencies. Actions from a RAID review often feed into a separate action log.

How often should an action log be reviewed?οΌ‹

Review it at every recurring meeting where the actions were raised, typically weekly or at each stand-up, to update statuses and close completed items.

Who is responsible for maintaining the action log?οΌ‹

Usually the project manager, scrum master, or meeting chair owns the log, but each action has its own named owner accountable for completing that specific item.
