About the Root Cause Analysis Fishbone template

A Root Cause Analysis Fishbone (also called an Ishikawa or cause-and-effect diagram) maps the potential causes of a defined problem across grouped categories so teams can move past symptoms to underlying drivers. Its branching structure forces structured brainstorming rather than jumping to the first obvious fix. It matters because durable corrective actions depend on identifying causes you can actually control, not just the ones that are visible.

It's part of My QMS, MyPMP's Quality Management System: fill it in online, personalize it with your name and logo, then export a clean, branded PDF. Your work auto-saves in your browser.

When to use a Root Cause Analysis Fishbone

  • β–ΈInvestigating a recurring defect, quality escape, or process failure that keeps returning after quick fixes
  • β–ΈRunning a team problem-solving session and needing to capture and organise many possible causes visually
  • β–ΈPreparing a CAPA, 8D, or post-incident review that requires evidence of structured cause analysis
  • β–ΈOnboarding a cross-functional group to a problem where causes span people, equipment, and process

What a good Root Cause Analysis Fishbone includes

  • βœ“Problem statement at the fish head: the specific effect or defect being analysed
  • βœ“Main cause categories on the spine, e.g. the 6Ms (Manpower, Method, Machine, Material, Measurement, Environment) or 4Ps for service work
  • βœ“Sub-causes branching off each category, drilled down with repeated 'why' questioning
  • βœ“Contributing factors distinguished from root causes so the diagram doesn't flatten priorities
  • βœ“A place to flag the most probable root causes for verification with data
  • βœ“Linked corrective or preventive actions tied to confirmed causes

Tips & common mistakes

  • πŸ’‘Keep asking 'why' on each branch until you reach a cause you can act on; stopping at a symptom is the most common failure
  • πŸ’‘Verify suspected root causes with data before committing actions, a fishbone generates hypotheses, not conclusions
  • πŸ’‘Don't overload one diagram, if the problem statement is broad, split it into separate analyses

How it works

  1. 1. Fill it in β€” type directly into the fields, tables and sections above.
  2. 2. Brand it β€” add your organization name and logo with the Branding button.
  3. 3. Export β€” print to PDF, or become a member to white-label and sync across devices.

FAQ

What are the standard fishbone categories?οΌ‹

Manufacturing commonly uses the 6Ms: Manpower, Method, Machine, Material, Measurement, and Environment (Mother Nature). Service and administrative work often use the 4Ps: People, Process, Policy, and Plant/technology.

How is a fishbone different from the 5 Whys?οΌ‹

The 5 Whys drills down a single causal chain, while a fishbone spreads analysis across multiple categories to capture many parallel causes. They work well together, use the fishbone to surface candidates, then 5 Whys to dig into each branch.

How many root causes should a fishbone identify?οΌ‹

There is no fixed number, but most problems trace back to a small set of two to four verified root causes. If you end up with a dozen, you likely haven't distinguished contributing factors from actual root causes yet.
