About the Audit Report template

An Audit Report documents the findings of a formal review against a defined standard, contract, or process, recording what was examined, what conformed, and where nonconformities or improvement opportunities exist. It gives auditees, management, and certification bodies an evidence-based record of compliance and the corrective actions required. Done well, it closes the loop between assessment and measurable process improvement.

It's part of My QMS, MyPMP's Quality Management System: fill it in online, personalize it with your name and logo, then export a clean, branded PDF. Your work auto-saves in your browser.

  • β–ΈReviewing supplier or subcontractor compliance against contract and quality requirements
  • β–ΈVerifying that a process, project phase, or gate meets documented procedures
  • β–ΈFollowing up on prior findings to confirm corrective actions have been implemented and are effective

Tips & common mistakes

  • πŸ’‘Tie every finding to a specific clause or requirement and cite the objective evidence β€” unsupported opinions weaken the report
  • πŸ’‘Distinguish clearly between nonconformities (must fix) and observations (should consider) so owners prioritise correctly
  • πŸ’‘Agree corrective action owners and dates before closing the audit, then track closure rather than filing the report and forgetting it

How it works

  1. 1. Fill it in β€” type directly into the fields, tables and sections above.
  2. 2. Brand it β€” add your organization name and logo with the Branding button.
  3. 3. Export β€” print to PDF, or become a member to white-label and sync across devices.

FAQ

What is the difference between a major and minor nonconformity?οΌ‹

A major nonconformity is a total breakdown or absence of a required control, or a systemic failure that undermines the process; a minor nonconformity is an isolated lapse that does not compromise the overall system.

Who is responsible for closing audit findings?οΌ‹

The auditee or process owner named for each finding is responsible for implementing corrective action, while the auditor verifies that the action is complete and effective before closure.

How should audit evidence be recorded?οΌ‹

Record objective, verifiable evidence such as document numbers, dates, record samples, or specific observations, so any finding can be independently traced and validated rather than relying on the auditor's judgement alone.
