About the RACI Matrix template
A RACI Matrix maps every task, deliverable, or decision in a project against the people involved, assigning each a role of Responsible, Accountable, Consulted, or Informed. It removes ambiguity about who does the work, who signs off, and who needs to be kept in the loop, which is where most cross-functional projects stall. Getting responsibilities agreed early prevents duplicated effort, missed handoffs, and decisions that get made twice.

Tips & common mistakes

FAQ
What is the difference between Responsible and Accountable in RACI?οΌ
Responsible people do the actual work to complete a task, while the Accountable person owns the outcome and has final sign-off authority. There should be one Accountable per task but there can be several Responsible contributors.
How many people can be Accountable for one task?οΌ
Only one. Assigning two or more Accountable roles to a single task splits decision authority and is the most common cause of stalled approvals.
What are RACI variations like RASCI or RACI-VS?οΌ
RASCI adds a Support role for those who assist the Responsible party, and RACI-VS adds Verify and Sign-off steps for quality and formal approval. They extend the base model when projects need finer distinctions.
