About the Performance Review template
A Performance Review is a structured evaluation of an individual's contribution against agreed goals, competencies, and behaviours over a defined period. It documents achievements, development needs, and next-cycle objectives, giving both manager and team member a shared record to guide pay, promotion, and growth decisions. Done well, it replaces vague impressions with evidence and clear expectations.
It's part of My QMS, MyPMP's Quality Management System: fill it in online, personalize it with your name and logo, then export a clean, branded PDF. Your work auto-saves in your browser.

FAQ
How often should performance reviews be held?οΌ
Most organisations run a formal review annually with a lighter mid-year check-in, though monthly or quarterly one-to-ones keep feedback current and prevent the annual review from becoming a surprise.
What rating scale works best for a performance review?οΌ
A 3- or 5-point scale with clearly defined labels (for example, below, meets, exceeds expectations) is common; the key is that each level has written criteria so ratings are consistent across reviewers.
Should the employee complete a self-assessment first?οΌ
Yes, a self-assessment before the meeting surfaces the employee's own view of achievements and gaps, prompts reflection, and often highlights accomplishments a manager may have missed.
