About the Issue Report template

An Issue Report documents a problem, query, or concern that has already occurred and needs a management decision or corrective action to keep the project on track. Unlike a risk, an issue is not a possibility but a current fact that affects scope, schedule, cost, or quality. Logging it formally ensures accountability, traceability, and a clear path to resolution.

A risk is an uncertain event that may happen in the future, while an issue is a problem that has already materialised and needs action now. Risks are managed in a risk register; issues are captured and resolved through an issue report or log.

Who is responsible for resolving an issue?οΌ‹

The project manager assigns an issue owner responsible for driving resolution, but decisions that exceed the manager's tolerance are escalated to the project board or sponsor.
