About the Change Request template
A Change Request is a formal proposal to alter an approved element of a project—scope, schedule, budget, deliverables, or requirements—so the impact can be assessed and authorised before any work happens. It creates an auditable record of who requested what, why, and how the change board decided, protecting the baseline from uncontrolled scope creep.
