A Transition Plan documents how ownership, knowledge, and responsibilities move from one party to another—typically from a project team to operations, from an outgoing vendor to an incoming one, or from a departing role-holder to a successor. It reduces the risk of dropped tasks, lost context, and service disruption by defining who does what, when, and to what acceptance criteria. Getting it right protects the value the project delivered once the project team disbands.

It is a document that sets out how responsibilities, knowledge, and assets are transferred from one team, role, or supplier to another. It covers scope, timeline, knowledge transfer, and the acceptance criteria needed to confirm a clean handover.
A handover is the moment of transfer; a Transition Plan is the structured approach that makes that handover succeed, including preparation, training, parallel running, and sign-off before and after the cutover date.
Usually a transition lead or project manager owns the plan, but it requires active commitment from both the outgoing owner who holds the knowledge and the incoming owner who must accept the work.
